The above-named person requests to be allowed hereby to participate in the activity or program stated.
The undersigned, for the sole and only consideration of participation in such activity or program, does hereby release, acquit and forever discharge the City of Sand Springs Park & Recreation Department and Board, the Sand Springs Municipal Authority and the City of Sand Springs, Oklahoma, its or their officers, agents, employees, volunteers, successors and assigns of and from any and all actions, causes of action, claims or demands for damage, costs, loss of services, expenses and compensation, on account of, or in any way growing out of, any loss, damages or injuries to persons or property which may occur during such participation in activities or programs of the Sand Springs Park & Recreation Department.
This release is full and complete as to all matters and things and claims arising from any such accident that may occur and is not to be construed as an admission of liability upon the part of the City of Sand Springs Park & Recreation Department or Board, the Sand Springs Municipal Authority, or the City of Sand Springs, Oklahoma, in the event of any such accident or injury.
The undersigned further certifies that as parents and guardian of the person named, if a minor or an infant, for and in consideration of the participation in the activity or program, which is provided to said minor at any request, do hereby remise, release, and forever discharge said City of Sand Springs Park & Recreation Department and Board, Sand Springs Municipal Authority, or the City of Sand Springs. Oklahoma, in its or their officers, agents, employees, volunteers, successors and assigns, of and from all liability, claims, or demands for any and all injuries to said minor arising or which may arise from any accident which may occur.
As further consideration for the providing of said activity/program, we hereby agree to hold harmless the said City of Sand Springs Park & Recreation Department and Board, Sand Springs Municipal Authority, and the City of Sand Springs, Oklahoma, its or their officers, agents, employees, volunteers, successors, and other person(s), for damages or compensation resulting in any way from any accident to said minor, and we also hereby agree to indemnify and said City of Sand Springs Park & Recreation and Board, Sand Springs Municipal Authority, City of Sand Springs, Oklahoma, its or their officers, agents, employees, volunteers, successors and assigns for any loss, damage or costs, including attorney fees, the said City of Sand Springs Park & Recreation Department and Board, the Sand Springs Municipal Authority, or the City of Sand Springs, Oklahoma, its or their officers, agents, employees, volunteers, successors and assigns, or any of them, incur or have to pay as the result of any such action, claim, or demand by the said minor or by any other person(s) on minor’s behalf or for minor’s benefit.
